Letter to request a recommendation letter

Dear Ms. Irin Cruz,

My name is Ernestina Gambrah. I am 16 years old in the 11th grade at HSES. I am writing this letter to ask you if feel as though you know me well enough to write me a letter of recommendation, in support of my college application(s).
Some other people that I plan to ask for letter's of recommendations include some teachers from my school ( Ms Brie. Valasek , Dr. Amit Basu and Ms. Shannon Hogan.)
